How to Leave Work at Work Again
One evening, I caught myself standing in my kitchen, staring into the refrigerator, trying to remember why I had opened it. Not because I was tired. Because I was still at work.
A patient I had seen that afternoon kept replaying in my mind. I was wondering whether I’d answered every portal message, whether I had forgotten to order a lab, and mentally organizing tomorrow’s clinic before dinner had even started.
I had been home for nearly an hour. My body had left the hospital. My mind hadn’t.
Over the years, I’ve realized this is one of the quietest ways medicine spills into our lives. It isn’t always the charts we finish after dinner or the inbox we check before bed. It’s the constant cognitive residue that follows us home. The unfinished conversations. The decisions we continue making long after the clinic ends. The feeling that we should always be available for just one more thing.
Most physicians assume this is simply what commitment looks like.
I don’t think it is.
The problem isn’t that we care too much. It’s that our careers have gradually lost their boundaries. Without realizing it, we’ve trained our brains to believe that physician mode is always on. That’s exhausting, even on days that don’t seem particularly busy.
One of the most valuable shifts I made wasn’t becoming more efficient. It was creating deliberate transitions out of work. Before leaving the clinic, I’d identify tomorrow’s three priorities, close every chart I reasonably could, and decide that the inbox could wait until morning. Those simple habits gave my brain permission to stop carrying unfinished work into the rest of my day.
What surprised me was what happened next. I didn’t become a less dedicated physician. I became more present everywhere else.
The goal isn’t to stop caring about your patients. The goal is to stop asking your mind to practice medicine twenty-four hours a day. Because a sustainable medical career isn’t built only by managing what happens during the clinic. It’s built by protecting the hours after the work day ends.
